USB keeps randomly disconnecting for G.I Ai 1300P PSU

I've had this problem since I bought the PSU but up until recently, I just can't get the USB to stay connected anymore. It will just disconnect and throw up a Code 43 error. Do I need a BIOS update or a new USB cable?

Yup, I've tried all 3 ports on the mobo, no difference at all.

Which mainboard you use?

Can you double check the USB cable? Make detailed pics of the connectors? Check for loose wire.
