I'm pretty sure you have to register your product on the MSI Member Center website. Once you have registered your product, it will ask you to complete the Questionnaire straight after it. If it didn't, you can go to My Products, select the product you just registered, and click Manage. Scroll all the way to the bottom and there is a Questionnaire link for it. It is what I did to get the extra 3 months' warranty for my MSI Laptop.
