Web Ticket form is broken

Not sure where else to post this, but the web ticket form (https://account.msi.com/en/services/na/inquiry) is broken. When you submit the form, nothing happens, and you can see these errors in the browser console:

Failed to load resource: the server responded with a status of 500 ()
Failed to load resource: the server responded with a status of 500 ()
Failed to load resource: the server responded with a status of 500 ()

JUst checked and it is working make sure you have the number correct no hash
